Introduction and objectives: Verapamil is a calcium antagonist that increases extracellular matrix collagenase secretion and decreases collagen and fibronectin synthesis and secretion, which could be useful in the acute stage of Peyronie Disease (PD). This stage is characterized by painful erection and penile curvature with or without difficulties for intercourse. Erectile dysfunction (ED) is sometimes associated. We evaluated the efficacy of intralesional Verapamil (ILV) in acute stage in order to improve or avoid worsening of pain and penile curvature and to know the adherence to the treatment. Material and method: From February 2002 to August 2007, 76 patients, (mean age 54.3 years old (range 31-70) with PD in acute stage were treated with ILV 10 mg (every two weeks/ 12 injections) under local anaesthesia (8cc Xylocaine 2% without epinephrine). Two months after the last dose, the patients answered a global assessment questionnaire (GAQ) in order to know if the treatment resulted effective to improve the symptoms associated with PD and we asked three questions: about pain, curvature and erectile function. Results: Sixty eight (91 %) patients completed the treatment, 7 dropped out and 1 is still on going. Overall the patients that completed the treatment ,74% answered afirmative the GAQ. Pain disappeared in 88% of the patients; penile curvature improved o stabilized in 86% and the erectile function improved or did not change in 91.5% of the patients. Conclusion: Intralesional verapamil was effective in order to stop or improve the symptoms associated with PD in accute stage. The adherence to the procedure was high.
